What Is Causing Your Neck and Shoulder Pain?

A Western Medicine and Traditional Chinese Medicine Perspective

Neck and shoulder pain is a common condition that can affect daily activities, sleep, and overall quality of life. Whether caused by prolonged computer use, poor posture, muscle strain, or underlying spinal conditions, understanding the source of discomfort is the first step toward effective pain management. Both Western Medicine and Traditional Chinese Medicine (TCM) offer valuable perspectives on managing these symptoms.
Why Does Neck and Shoulder Pain Occur?
From a Western medical perspective, the cervical spine consists of seven vertebrae supported by intervertebral discs, joints, ligaments, muscles, and nerves. The trapezius, levator scapulae, and surrounding muscles help stabilize the neck and shoulders while allowing movement.
Prolonged sitting, repetitive movements, muscle overuse, and psychological stress may increase muscular tension and contribute to pain. Other possible causes include cervical osteoarthritis, herniated discs, nerve compression, and injuries such as whiplash. Symptoms may include stiffness, headaches, restricted movement, or pain radiating into the arms. Numbness, tingling, or weakness may indicate nerve involvement and require medical evaluation. [1–3]
Understanding Neck and Shoulder Pain Through TCM
Traditional Chinese Medicine views neck and shoulder discomfort through patterns involving the circulation of Qi and Blood along the body's meridians. Pain may be interpreted as Qi and Blood stagnation, Cold-Damp obstruction, or other patterns identified through an individualized TCM assessment.
In TCM theory, prolonged tension, emotional stress, and exposure to cold may contribute to these patterns. Acupuncture is traditionally used to regulate Qi, promote the movement of Blood, and relieve discomfort. These concepts represent a traditional diagnostic framework rather than established anatomical mechanisms.
Clinical research has investigated TCM approaches for chronic neck and shoulder pain, although findings vary according to treatment methods and study quality. [4–5]
How Can You Relieve and Prevent Neck and Shoulder Pain?
For uncomplicated muscular discomfort, gentle movement, appropriate stretching, and regular breaks from prolonged sitting may help reduce stiffness. Maintaining a comfortable workstation, keeping screens near eye level, and strengthening the neck, shoulder, and upper-back muscles can support better posture and movement.
At home, warm compresses may help relax tight muscles, while cold packs may provide temporary relief after a recent minor injury. Avoid forceful neck manipulation, aggressive stretching, prolonged bed rest, and activities that significantly worsen symptoms. Gentle activity is generally preferable when tolerated. [2–3]
Nutrition also supports musculoskeletal health. A balanced diet containing adequate protein, calcium, vitamin D, magnesium, and vitamin B12 helps maintain normal muscle, bone, and nerve function. Foods such as fish, leafy vegetables, legumes, nuts, eggs, and fortified products can provide important nutrients. However, vitamins and supplements are not established cures for mechanical neck pain, and supplementation should be based on individual nutritional needs.
From a TCM perspective, regular meals, adequate rest, gentle exercise, and keeping the neck comfortably warm may be recommended as supportive lifestyle practices. Herbal remedies should only be considered after professional evaluation because they may interact with medications.
When Should You Seek Medical Attention?
Although many cases of neck and shoulder pain improve with conservative care, persistent or worsening symptoms should be evaluated by a qualified healthcare professional. Seek urgent medical attention for significant trauma, progressive arm weakness, difficulty walking, fever with severe neck stiffness, or neck and shoulder discomfort accompanied by chest pain or shortness of breath. [2–3]
How Can Acupuncture at Caulo Care Help?
Acupuncture may offer a nonpharmacological option for managing certain types of neck and shoulder pain, particularly persistent musculoskeletal discomfort.
A 2023 retrospective study published in the American Journal of Translational Research reported greater improvements in pain and functional outcomes among patients receiving acupuncture combined with other TCM treatments and conventional medical care compared with conventional treatment alone. However, because multiple treatments were combined and the study was observational, the findings cannot establish the independent effectiveness of acupuncture. [5]
At Caulo Care Acupuncture in Forest Hills, Queens, we take an individualized approach to neck and shoulder discomfort, considering symptoms, movement limitations, lifestyle factors, and TCM assessment findings. Our goal is to help patients manage pain, reduce muscular discomfort, and support comfortable daily movement. Acupuncture may be incorporated into a broader care plan alongside appropriate medical evaluation, exercise, and self-care. Results vary, and acupuncture does not replace necessary treatment for structural injuries or neurological conditions.
Don't let neck and shoulder pain continue to interfere with your daily life. Discover whether acupuncture may be an appropriate option for your pain management plan at Caulo Care Acupuncture.
